Registering is as easy as 1,2,3! All three steps are clearly outlined below in the order they should be followed. Following all the instructions will speed up the process and ensure that there is no delay in processing your membership.

Step One - The Forms:
All forms are available for download in PDF format (requires Adobe Acrobat to view). If you don't currently have Acrobat on your computer, click here to download your free version.

1. Complete the Membership Application Form. Note:  Some forms require a witness signature.
   
2. If you answered "Yes" to any question in the ParQ questionnaire, please print and have the Medical Release Form completed by your physician.
   
3. Complete the Pre-authorized Payment Agreement. 
   
   

Please submit all forms to the Fitness Centre Co-ordinator to begin processing your membership.

Step Two - The Orientation Session:
Once you complete all the membership registration forms you will be required to participate in a facility orientation session.  The orientation session is your introduction to the facility, equipment, policies and procedures.

Step Three - Registration:
Once you have completed all the above, bring completed forms plus the initial 3 month membership and initiation fee payment of $123.90 (GST incl.) to the Fitness Centre (please note post-dated cheques will not be accepted).  After the 3-month minimum commitment, the monthly membership fee is $26/mth (plus GST), subject to change.  The Fitness & Wellness Co-ordinator will then book your Fitness Assessment.  Note:  The Fitness Assessment is not mandatory to join.
